If your name of record does not match your name exactly as it appears on your current, unexpired passport, you must follow our established process to request a change of name in your record. You will be required to provide a reason for the name change, as well as supporting documentation. Additional information and instructions are provided via MyIntealth.
Important Note: You are responsible for maintaining a current name of record with us and with FSMB. Changing your name with one entity will not automatically update your name with the other entity. Please refer to FSMB for more information on its process for changing your name.
If you have a valid identification form on file with us, it will be invalidated when your name of record is changed, and you will be required to complete a new Intealth Identification Form before you may submit applications/service requests.
